      • Child's Name:
      • Child's Age:
      • Preferred YMCA location:
      • Requested Camp Week(s) including Dates:
      • Any other information we need to know?
  2. YMCA staff will respond and send you the required paperwork
  3. Fill out paperwork and submit by email
  4. YMCA staff will review and once approved, will register your child
  5. Payment to be made by e-transfer
